ABSTRACT

RESUMO Objetivo: avaliar o perfil de segurança e os resultados estéticos do 2-octilcianoacrilato versus sutura intradérmica com fio de nylon em cirurgias mamárias. Métodos: ensaio clínico randomizado, aberto, que avaliou a ocorrência de complicações, como deiscência, hematoma, infecção e reações alérgicas após o uso do 2-octilcianoacrilato ou do fio de nylon. Também foi analisado o tamanho das incisões, o tempo de fechamento da pele e o tempo cirúrgico total. O resultado estético foi avaliado após 40 e 180 dias da cirurgia, por meio da largura média da ferida operatória e por avaliação subjetiva conceitual (ótimo, bom, razoável ou ruim). Resultados: foram incluídas 79 pacientes, sendo 37 no grupo 2-octilcianoacrilato e 42 no grupo de sutura com fio de nylon. O estudo foi interrompido antes do término do recrutamento dos pacientes pela ocorrência de maior número de deiscências no grupo do adesivo (OR: 11,42; IC95%: 1,36-96,02; p=0,007). Em relação às demais complicações analisadas, ao tempo cirúrgico e ao resultado estético no pós-operatório, não se observaram diferenças significativas entre os grupos. A média do tamanho da ferida operatória foi maior no grupo do adesivo em relação ao grupo da sutura, mas não houve correlação entre o tamanho da ferida e o maior número de deiscências. Conclusão: o 2-octilcianoacrilato apresentou maior risco de deiscência em relação à sutura intradérmica, com resultados estéticos equivalentes.


ABSTRACT Objective: to evaluate the safety profile and aesthetic results of 2-octyl-cyanoacrylate versus intradermal nylon suture in breast surgeries. Methods: an open-label, randomized, clinical trial evaluating the occurrence of complications, such as dehiscence, hematoma, infection, and allergic reactions after the use of 2-octyl-cyanoacrylate or nylon thread. The size of the incisions, skin closure time, and total surgical time were also analyzed. The aesthetic outcome was evaluated at 40 and 180 days after surgery, by means of the average width of the surgical wound and by subjective conceptual assessment (optimal, good, reasonable, or poor). Results: 79 patients were included: 37 in the 2-octyl-cyanoacrylate group and 42 in the nylon suture group. The study was stopped before the end of patient recruitment due to the occurrence of a greater number of dehiscences in the adhesive group (OR: 11.42; 95%CI: 1.36-96.02; p=0.007). Regarding the other analyzed complications, the surgical duration and postoperative aesthetic result, no significant differences were observed between the groups. The mean operative wound size was greater in the adhesive group than in the suture group, but there was no correlation between wound size and the largest number of dehiscences. Conclusion: while the cosmetic outcomes with the two techniques were similar, there was a greater risk of dehiscence with the use of 2-octyl-cyanoacrylate compared to intradermal suturing.

ABSTRACT Aim: To evaluate rupture pressures of tissue adhesives of cyanoacrylate (Omnex®) and fibrin (Evicel®), used as reinforcement in colonic suture from "ex vivo" swine. Methods: Surgical procedures were performed in the Surgical Technique Laboratory. From a division in segments of 10 cm of descending colon and sigmoid colon from three "ex vivo" female swine, Landrace breed, which were resected in less than six hours after the slaughter time, 30 segments were selected, 10 of each animal. They were stored in saline solution 0.9% at 36 °C, being randomly allocated in three groups (Control, Evicel and Omnex), each one containing 10 segments. Results: The lower and higher pressure values found in the groups Control, Evicel and Omnex were 36 mmHg and 41 mmHg, 70 mmHg and 90 mmHg, 90 mmHg and 120 mmHg, respectively. Containing statistical significance (p-value <0.0001) concerning the 2 to 2 comparisons (Control, Evicel and Omnex) with 95% trusting rate based on the application of the Turkey Method. Conclusion: One concludes that the use of tissue adhesives in anastomoses colonic in an experimental animal model of "ex vivo" swine increased the anastomoses rupture pressures. Among the tested adhesives, cyanoacrylate presented higher rupture pressure in relation to fibrin adhesive.


RESUMO Objetivo: Avaliar as pressões de ruptura dos adesivos teciduais de Cianoacrilato (Omnex®) e de Fibrina (Evicel®), usados como reforço em suturas colônicas de suínos "ex-vivo". Métodos: Os procedimentos cirúrgicos realizados foram realizados no Laboratório de Técnica Cirúrgica. A partir da divisão em segmentos de 10 cm do colo descendente e colo sigmoide de três suínas fêmeas ex-vivo, da raça Landrace, ressecados em tempo inferior a seis horas em relação ao momento do abate, foram selecionados 30 segmentos, 10 de cada animal. Foram armazenados em soro fisiológico 0,9% a 36 °C, alocando-se aleatoriamente esses segmentos em três grupos (Controle, Evicel e Omnex) com 10 segmentos cada. Resultados: Os menores e maiores valores pressóricos encontrados nos grupos Controle, Evicel e Omnex foram 36 mmHg e 41 mmHg, 70 mmHg e 90 mmHg, 90 mmHg e 120 mmHg, respectivamente. Com significância estatística (Valor-p < 0,0001) para as comparações 2 a 2 (Sutura, Evicel e Omnex) com um intervalo de confiança de 95% construído a partir da aplicação do método de Turkey. Conclusão: A partir desse estudo conclui-se que o uso de adesivos teciduais em anastomoses colônicas, em modelo experimental animal de suíno ex-vivo, aumentou as pressões de ruptura das anastomoses. Dentre os adesivos testados, o adesivo de Cianoacrilato apresentou maiores pressões de ruptura em relação ao adesivo de Fibrina.

ABSTRACT

OBJECTIVES: Tissue adhesives can be used to prevent pulmonary air leaks, which frequently occur after lung interventions. The objective of this study is to evaluate local and systemic effects of fibrin and cyanoacrylate tissue adhesives on lung lesions in rabbits. METHODS: Eighteen rabbits were submitted to videothoracoscopy + lung incision alone (control) or videothoracoscopy + lung incision + local application of fibrin or cyanoacrylate adhesive. Blood samples were collected and assessed for leukocyte, neutrophil and lymphocyte counts and interleukin-8 levels preoperatively and at 48 hours and 28 days post-operatively. After 28 days, the animals were euthanized for gross examination of the lung surface, and lung fragments were excised for histopathological analysis. RESULTS: Fibrin and cyanoacrylate produced similar adhesion scores of the lung to the parietal pleura. Microscopic analysis revealed uniform low-cellular tissue infiltration in the fibrin group and an intense tissue reaction characterized by dense inflammatory infiltration of granulocytes, giant cells and necrosis in the cyanoacrylate group. No changes were detected in the leukocyte, neutrophil or lymphocyte count at any time-point, while the interleukin-8 levels were increased in the fibrin and cyanoacrylate groups after 48 hours compared with the pre-operative control levels (p<0.01). CONCLUSION: Both adhesive agents promoted normal tissue healing, with a more pronounced local inflammatory reaction observed for cyanoacrylate. Among the serum markers of inflammation, only the interleukin-8 levels changed post-operatively, increasing after 48 hours and decreasing after 28 days to levels similar to those of the control group in both the fibrin and cyanoacrylate groups.

ABSTRACT

Management of gastrointestinal bleeding caused by fundal varices is particularly difficult to manage. The options are: transjugular intrahepatic portosystemic shunt (TIPS), endoscopic injection of cyanoacrylate or balloon-occluded retrograde transvenous obliteration (BRTO). We report a 63 year-old male with a cirrhosis caused by hepatitis C and a 66 year-old female with a cirrhosis caused by a non-alcoholic steatohepatitis. Both patients had a gastrointestinal bleeding caused by fundal varices and were treated with sclerotherapy with cyanoacrylate assisted with BRTO. Flow was interrupted in the gastro-renal shunt by a femoral access in both patients. The male patient had a new bleeding two months later and died. In the female patient an endosonography performed nine months after the procedure showed absence of remaining varices.

ABSTRACT

RESUMEN: Los adhesivos de cianoacrilatos (ACA) son materiales sintéticos con propiedades adhesivas. Al ser aplicados en los tejidos polimerizan uniéndose con el tejido subyacente. Desde la década de los 70' se han explorado sus aplicaciones quirúrgicas para el cierre de heridas y fístulas, control de sangrado y fijación de injertos, entre otros, siendo su uso como alternativa para el cierre de heridas en piel y mucosas uno de los más estudiados. Los ACA presentan un limitado grado de absorción, sin evidencia de efectos tóxicos sistémicos. Tienen la ventaja de ser aplicados de forma rápida, indolora, con efecto antibacteriano y hemostático según los reportes de la literatura, pero presentan una reducida fuerza de tensión. El objetivo de esta revisión de la literatura es describir los usos y aplicaciones de los ACA, con enfoque en la cirugía oral y maxilofacial, evaluando de forma crítica sus aplicaciones.


ABSTRACT: The cyanoacrylate adhesives (ACA) are synthetic materials with adhesive properties. When is applied in tissues, it polymerizes and bonds with the underlying tissue. Since the 70s' have been explored their surgical applications for closing wounds, fistulas, bleeding control, and graft fixation, among others. Its use as an alternative for closing wounds in skin and mucous is one of the most studied. The ACA have a limited absorption degree, with no evidence of systemic toxic effects. They have the advantage of being applied quickly, painlessly, with antibacterial effect and hemostatic according to the report of literature, but with reduced tensile strength. The objective of this literature review is to describe the use and applications of ACA, with focus on oral and maxillofacial surgery, with a critically evaluation of their applications.

Abstract Purpose: To compare the use of a new cyanoacrylate-based surgical glue and suture with sepa-rate points in skin wounds closure. Methods: Thirty-six rats were subjected to a 4cm dorsal longitudinal incision. Twelve were sub-jected to simple suture with polyamide 6-0, 12 rats underwent wall synthesis using Dermabond(r) and 12 was performed cutaneous synthesis with N-2-Butyl-Cyanoacrylate. Twelve of each group was euthanized on the seventh postoperative day, their blood was taken to biochemical tests and a layer of skin and subcutaneous tissue surrounding the surgical scar was randomly divided in two segments, to the submission of tension tests and to histological study. Results: There were no significant difference between groups (p>0.05). In the soft dermis there was more type I collagen production in group I (p<0.05), group II and III was similar re-sults (p>0.05). In the compact dermis, all 3 groups showed similar results (p>0.05). The biomechanical study was similarity between the glue groups (p>0.05) but the group III proved to be different from the others having a higher resistance (p>0.05) . Conclusion: This glue does not cause any inflammation or kidney and hepatic toxicity. Polyamide sutures are more resistant and the glue should be used alone only in less tension are-as.

ABSTRACT PURPOSE: To evaluate the efficacy of ethyl-cyanoacrylate in the treatment of fístula-in-ano in rats with and without prior seton placement. METHODS: Thirty rats Wistar with fístula-in-ano produced surgically, distributed in three groups: group A (ethyl-cyanoacrylate) - treated by application of ethyl-cyanoacrylate into fístula tract; group B (seton + ethyl-cyanoacrylate) - seton placement followed by application of ethyl-cyanoacrylate into fístula tract, 30 days later; group C (control) - no treatment. After 60 days the animals were submited to euthanasia and the specimens were analyzed by pathologist. The results were analyzed by Chi-square test with significant value of p<0.05. RESULTS: One animal of group B died. Were found tracts fully healed: 7, 5 and 2, in groups A, B and C, respectivelly. There was a statistically significant difference between groups A and C (p=0.02). When joined all animals treated with glue (A + B) compared to group C there was also statistically significant difference (p=0.02). CONCLUSIONS: The use of ethyl-cyanoacrylate glue was effective in closing anal fistulae in rats. There was no advantage in the prior application of seton.

ABSTRACT Background Circumcision is performed as a routine operation in many countries, more commonly for religious and cultural reasons than for indicated conditions, such as phimosis and balanitis. There are many techniques available, and recently electrocautery and both Nd:YAG and CO2 lasers, instead of blades, have been used for skin and mucosal incisions. However, the infection risk in circumcisions performed using a CO2 laser was 10% higher. There are also reports of sutureless procedures using cyanoacrylate, but these have higher risks of hematoma and hemorrhage. We combined a CO2 laser and cyanoacrylate to shorten the operation time and to decrease bleeding complications. Materials and Methods : Circumcisions were performed under general anesthesia with CO2 laser and cyanoacrylate combination in 75 6–9-year-old boys between May 2013 and August 2014 only for religious reasons. As a control, we compared them retrospectively with 75 age-matched patients who were circumcised using the conventional guillotine method in our clinic. Results No hematomas, bleeding, or wound infections were observed. One wound dehiscence (1.33%) occurred during the early postoperative period and healed without any additional procedures. The median operating time was 7 (range 6–9) minutes. The conventional guillotine group comprised one hematoma (1.3%), two wound dehiscences (2.6%), and two hemorrhages (2.6%), and the median operating time was 22 (range 20–26) minutes. The difference in surgical time was significant (p<0.001), with no significant difference in the rate of complications between the two groups. Conclusion The combined CO2 laser and cyanoacrylate procedure not only decreased the operating time markedly, but also eliminated the disadvantages associated with each individual procedure alone.

ABSTRACT

O uso de adesivos teciduais à base de cianoacrilato como alternativa às suturas mecânicas tem despertado grande interesse entre os cirurgiões, pela promoção de uma adesão de qualidade dos tecidos em tempo reduzido e, ainda, por suas propriedades hemostáticas e bacteriostáticas. Neste contexto, a presente revisão discute as propriedades dos diversos tipos de adesivos teciduais e suas indicações nas cirurgias da parede abdominal.


The use of cyanoacrylate-based tissue adhesives as an alternative to surgical stapling has arisen great interest among surgeons, for promoting quality tissue adhesion in reduced time, and also for its hemostatic and bacteriostatic properties. In this context, this review discusses the properties of various types of tissue adhesives, and their indications for surgery of the abdominal wall.

ABSTRACT

INTRODUÇÃO: O uso de ornamentos auriculares é uma prática multicultural. Considerando a grande incidência de fissuras de lóbulo de orelha (FLOs) no mundo inteiro, é surpreendente a escassez de avaliações objetivas na literatura nacional e internacional. OBJETIVO: Avaliação dos resultados do fechamento de FLO's por meio de simples remoção de suas bordas e fechamento usando adesivo tissular. MÉTODOS: Estudo prospectivo no qual FLOs (36) foram tratadas por simples excisão de suas bordas epitelizadas e fechamento usando cianoacrilato como adesivo tissular. RESULTADOS: Foram tratadas FLOs causadas por brincos (91,7%) e por trauma (8,3%). Achatamento (14,8%), cicatriz inestética (13,9%), entalhe (11,1%), alongamento lobular (7,4%) e deiscência (5,6%) foram os problemas encontrados. Não houve casos de necrose ou queloides. CONCLUSÕES: O tratamento proposto mostrou-se seguro e com resultados cosméticos satisfatórios em 92,6% das pacientes.


INTRODUCTION: The use of ear ornaments is a multicultural practice. The lack of objective evaluations published in the national and international literature is surprising considering the high incidence of torn earlobe (TEL) worldwide. OBJECTIVE: Evaluation of the results of torn earlobe repair with simple marginal excision and closure using a tissue adhesive. METHODS: This was a prospective study in which TELs (36) were treated by simple excision of epithelialized edges and closure using cyanoacrylate as tissue adhesive. RESULTS: TELs caused by earrings (91.7%) and trauma (8.3%) were treated. The following issues were found: flatness (14.8%), unsightly scar (13.9%), notching (11.1%), stretched earlobe (7.4%), and dehiscence (5.6%). No cases of necrosis or keloid scars occurred. CONCLUSIONS: The proposed treatment proved safe and had satisfactory cosmetic results in 92.6% of patients.

ABSTRACT

BACKGROUND/AIMS: Practice guidelines recommend endoscopic band ligation (EBL) and endoscopic variceal obturation (EVO) for bleeding from esophageal varices and fundal varices, respectively. However, the optimal treatment for bleeding from cardiac varices along the lesser curvature of the stomach (GOV1) remains undefined. This retrospective study compared the efficacy between EBL and EVO for bleeding from GOV1. METHODS: Patients treated by EBL or EVO via cyanoacrylate injection for bleeding from GOV1 were enrolled. Patients diagnosed with hepatocellular carcinoma or treated with endoscopic injection sclerotherapy were excluded. RESULTS: The study included 91 patients treated for bleeding from GOV1. The mean age was 56.3±10.9 years (mean±SD), and 78 of them (85.7%) were men. Overall, 51 and 40 patients were treated with EBL and EVO, respectively. A trend for a higher hemostasis rate was noted in the EVO group (100%) than in the EBL group (82.6%, P=0.078). Varices rebled in 15 patients during follow-up. The rebleeding rate was significantly higher in the EBL group than in the EVO group (P=0.004). During follow-up, 13 patients died (11 in the EBL group and 2 in the EVO group); the survival rate was marginally significant between two groups (P=0.050). The rebleeding-free survival rate was significantly higher in the EVO group than in the EBL group (P=0.001). CONCLUSIONS: Compared to EBL, EVO offered significantly lower rebleeding rates, significantly higher rebleeding-free survival rates, and a trend for higher hemostasis and survival rates. EVO appears to be the better therapeutic option for bleeding from GOV1.

ABSTRACT

Os adesivos a base de cianoacrilato são utilizados em cirurgia oral e maxilofacial para fechamento de feridas cirúrgicas, comunicação buco-sinusal, reparo de fissura labial e osteosíntese do esqueleto craniofacial. O objetivo deste trabalho foi comparar três métodos de fixação (etil-cianoacrilato, n-butil-cianoacrilato ou parafuso de titânio) do enxerto ósseo autógeno (EOA) em bloco. Foram realizadas ostectomias bilaterais na região parietal em trinta coelhos com uma trefina de 6mm para obtenção do EOA. Os EOA em bloco obtidos foram fixados com etil-cianoacrilato (Grupo I), n-butil-cianoacrilato (Grupo II) ou parafuso de titânio (Grupo III). Os animais foram sacrificados nos períodos de cinco, quinze, trinta, sessenta e cento e vinte dias, sendo obtidas amostras da região do EOA. As peças foram processadas em laboratório de acordo com o método histotécnico de rotina e coradas com hematoxilina e eosina. Os eventos histológicos avaliados foram: infiltrado inflamatório, reabsorção do adesivo e neoformação e reabsorção óssea. Os resultados dos eventos celulares foram submetidos ao teste de Kruskal-Wallis, seguido pelo teste de Dunn para determinar as diferenças entre os grupos (p < 0.05). A análise qualitativa ordinal mostrou a presença de infiltrado inflamatório e reabsorção óssea mais significativos nos grupos I e II, principalmente nos períodos iniciais. O infiltrado inflamatório observado no grupo controle foi discreto (p<0.05). O processo de reabsorção do adesivo foi semelhante, não havendo diferença estatística (p>0.05). No período de 120 dias, foi possível observar adesivo na interface EOA-leito receptor. A neoformação óssea foi mais intensa no grupo controle e no período de 120 dias, o EOA em bloco estava totalmente incorporado ao leito receptor. Cyanoacrylates are used in oral and maxillofacial surgery for closure of surgical wounds, oroantral communication, cleft lip repair, and osteosynthesis of the craniofacial skeleton. The aim of this study was compare three fixation methods (ethyl-cyanoacrylate, n-butyl cyanoacrylate or titanium screw) of autogenous onlay bone graft (AOBG). Bilateral ostectomies were made in the parietal region in thirty rabbits using a 6-mm-diameter trephine. The AOBG was fixed with ethyl-cyanoacrylate (Group I), n-butyl cyanoacrylate (Group II) or titanium screw (Group III - control). The animals were sacrificed at five, fifteen, thirty, sixty, and one hundred and twenty days, and the samples were obtained from AOBG region. The processing was done in accordance with the method histotechnical and stained with hematoxylin and eosin. The evaluated histological events were: inflammatory infiltrate, adhesive resorption and bone formation and bone resorption. The results of cellular events were subjected to the Kruskal-Wallis test followed by Dunn’s test to determine differences between groups (p <0.05). The qualitative numerical analysis showed the presence of inflammatory infiltrate and significant bone resorption in groups I and II, especially in the early periods. The inflammatory infiltrate seen in the control group was discrete (p <0.05). The adhesive reabsorption process was similar, with no statistical difference (p> 0.05). After 120 days, we observed residual adhesive between AOBG and host bed. Newly formed bone was more intense in the control group and, at period 120 days the AOBG was completely incorporated into the host bed. In groups I and II, bone formation was less intense in group III (p<0.05 ). The titanium screw was the best method of fixation of the AOBG. However, adhesives ethyl and n-butyl cyanoacrylate are viable alternative to fixation from AOBG.

ABSTRACT

The objectives of this study were to compare surgical techniques and the effects of using n-butyl 2-cyanoacrylate and bovine amniotic membrane to repair perforated lesions in corneas. Penetrating keratoplasty was performed in sixty New Zealand White rabbits under general anesthesia. Group 1 (G1) was treated with n-butyl 2-cyanoacrylate, group 2 (G2) received a fragment of amniotic membrane through the anterior chamber and application of n-butyl 2-cyanoacrylate over the lesion, group 3 (G3) was treated with the same technique as G2 with the addition of an amniotic membrane bandage covering the cornea and sutured in the limbus region, and group 4 (G4) was treated with an amniotic membrane sutured to the lesion and an amniotic membrane bandage sutured in the limbus region. Clinical, histological and histomorphometric examinations of the corneas were performed. The membrane acted as a barrier for aqueous humor in G2 and G3, thereby keeping the surface dry for adhesive application; it also prevented the adhesive from contacting intraocular structures. The groups treated with amniotic membrane and surgical adhesive showed better results than the groups treated with either material alone. Os objetivos deste estudo foram comparar técnicas cirúrgicas e efeitos do n-butil 2-cianoacrilato e da membrana amniótica bovina na reparação de lesões perfuradas em córneas. Sessenta coelhos da raça Nova Zelândia Branca foram submetidos à ceratoplastia penetrante sob anestesia geral. O grupo 1 (G1) foi tratado com n-butil 2-cianoacrilato; o grupo 2 (G2) recebeu um fragmento de membrana amniótica pela câmara anterior e aplicação de n-butil 2-cianoacrilato sobre a lesão; o grupo 3 (G3) foi tratado com a mesma técnica aplicada ao G2, adicionando-se uma bandagem de membrana amniótica cobrindo a córnea e suturada à região do limbo; e o grupo 4 (G4) foi tratado com membrana amniótica suturada nas bordas da lesão e bandagem de membrana amniótica suturada na região do limbo. Foram realizados exames clínico, histológico e histomorfométrico. A membrana atuou como barreira contra o extravasamento do humor aquoso nos grupos 2 e 3, manteve a superfície seca para posterior aplicação do adesivo e impediu o contato do adesivo com as estruturas intraoculares. ABSTRACT

PURPOSE: To evaluate wound leakage and bacteria-sized particle influx in differently corneal sealed side port incisions. METHODS: Four 1.5mm tunnel squared incisions were created in each of four cadaveric human eyes. In each cornea, one incision was left unsealed, whereas the other three incisions were sealed using a 10-0 nylon suture, cyanoacrylate glue, or stromal hydration, respectively. A Seidel and an India ink test were performed on each eye. During each Seidel test, flourescein was applied, the IOP increased from 15 to 80mmHg, and the IOP at which each incision started to leak recorded. During each India ink test, ink was placed on the eye and rinsed out with balanced salt solution (BSS). Ink penetration was then measured by planimetry at physiologic conditions and after an IOP plunge from 80mmHg to 0mmHg. RESULTS: Regardless of IOP variations, no leakage or ink inflow was observed through the glued incisions. In contrast, leakage did occur in the other three sealing methods, albeit at significantly different IOP levels in each one (p=0.013). Ink inflow occurred in these sealing methods at physiologic IOP and, to a significantly greater extent, after the IOP challenge (p<0.05). At both of these IOP conditions, the differences in ink influx among these three incision-sealing methods were deemed statistically insignificant. CONCLUSION: This study showed that glue was more effective at preventing wound leakage and bacteria-sized particle influx than other commonly used methods especially hydrosealing.


OBJETIVO: Avaliar o sinal de Seidel positivo e a penetração de tinta da Índia em incisões corneanas acessórias seladas por diferentes métodos. MÉTODOS: Quatro incisões de 1,5 x 1,5mm foram criadas em cada um dos quatro olhos provindos do banco de olhos. Em cada córnea, uma incisão foi mantida sem manipulação, enquanto que as outras três incisões foram seladas respectivamente com Nylon 10-0, cola de cianoacrilato e hidratação estromal. Foram realizados dois testes: Sinal de Seidel positivo e penetração da tinta da Índia. No primeiro, foi aplicado fluoresceína gotas e a pressão intraocular (PIO) elevada de 15 para 80mmHg. No segundo, a tinta da Índia foi aplicada sobre o olho estudado em duas situações: sob PIO fisiológica e sob variação súbita da pressão, de 80 para 0mmHg. RESULTADOS: Na incisão selada com cola, apesar da variação da PIO, não houve vazamento e nem penetração de partículas de tinta. Por outro lado, o sinal de Seidel foi positivo nas outras três incisões em diferentes níveis de PIO (p=0,013). A penetração da tinta da Índia ocorreu nestas três incisões sob pressão fisiológica e com maior extensão após a variação da PIO (p<0,05). Esta diferença, no entanto não foi considerada estatisticamente significante quando comparadas as incisões entre si. CONCLUSÃO: No presente estudo, a cola foi mais eficaz em prevenir Seidel e entrada de partículas do que outro método comumente usado especialmente, hidratação estromal.

ABSTRACT

Purpose The aim of this video is to demonstrate an endoscopic and minimally invasive repair of an urethrocutaneous fistula with cyanoacrylate glue. Materials and Methods: A 56 year-old-man with post-infectious urethral stricture and recurrent perineal abscess formation due to urethral fistulas. Results The operative time was 60 minutes, no major complications were observed perioperatively and postoperatively. At a follow-up time of 6 months the patient had no evidence of recurrent fistula and abscess formation. CONCLUSIONS The endoscopic use of cyanoacrylate glue represents a safe and minimally invasive approach that might be offered as a first line option for the treatment of urinary fistulas in selected patients, especially those with narrow and long tracts. .

ABSTRACT

Cyanoacrylate has been used in several fields of different surgical specialties as an adhesive for closure of gingival flaps and in mucous and cutaneous lacerations. One of its advantages is that it has an excellent immunological response. In view of aesthetic needs, cyanoacrylate has been applied with satisfactory results, when compared with sutures. It presents better coaptation of edges of cutaneous and mucosal lesions, smaller residual scars, and biocompatibility. However, it is limited to areas of little tissue tension. This work attempts to provide a literature review with the aim of revealing the advantages of using tissue adhesives, especially cyanoacrylates, in wound coaptation in comparison with conventional methods.


O cianoacrilato tem sido utilizado em diversos campos das especialidades cirúrgicas como adesivo no fechamento de retalhos gengivais e em lacerações mucosas e cutâneas, além de apresentar uma ótima resposta imunológica. Tendo em vista as necessidades estéticas, o cianoacrilato tem sido aplicado com resultados satisfatórios quando comparado aos fios de sutura, pois apresenta melhor capacidade de coaptação das bordas de uma lesão de pele e mucosa, menor cicatriz residual e biocompatiblidade, estando limitado, entretanto, a zonas de baixa tensão tecidual. Frente a tais considerações, o presente trabalho busca desenvolver uma revisão literária, objetivando revelar as vantagens do uso dos adesivos teciduais, especialmente os cianoacrilatos, na coaptação de feridas, em detrimento da utilização dos métodos convencionais com fios de sutura.

ABSTRACT

PURPOSE: To compare the wound healing of the abdominal wall of rabbits exposed to nicotine and submitted to abdominoplasty using 2-octyl cyanoacrylate or nylon thread for the surgery suture. METHODS: Thirty two rabbits were used. They were divided in subgroups: A1, A2, B1 e B2. Group A received saline 0.9%; group B received nicotine, both groups for 14 days before surgery. We performed an abdominoplasty with a nylon suture into the A1 and B1 subgroups; as for A2 and B2 groups the suture was performed with cyanoacrylate. The euthanasia happened in the 14th post-operative day. After, we evaluated: swollen process, fibroblast proliferation, collagen, neovascularization, and macroscope and microscope epithelization of the scars. RESULTS: We observed the presence of eosinophils in all scars exposed to the cyanoacrylate, and a significant increase of neovascularization in the subgroup B2 comparing to the A2 one (p=0.037). The other variables haven't showed any statistical difference. CONCLUSIONS: Nicotine hasn't influenced the swollen process, the fibroblast proliferation, the presence of collagen, neither the epithelialization. The neovascularization showed cicatricial immaturity when comparing group A2 to group B2. The eosinophils in the scars repaired with glue showed that the substance has acted as an allergen.


OBJETIVO: Comparar a cicatrização da parede abdominal de coelhos expostos à nicotina e submetidos à abdominoplastia utilizando 2-octil cianoacrilato ou nylon na síntese cirúrgica. MÉTODOS: Utilizou-se 32 coelhos. Estes foram distribuídos em subgrupos: A1, A2, B1 e B2. O grupo A recebeu solução de NaCl 0,9%; o B recebeu nicotina, ambos durante 14 dias do pré-operatório. Nos subgrupos A1 e B1 foi realizada abdominoplastia e sutura com "nylon"; enquanto A2 e B2 a síntese ocorreu com cianoacrilato. A eutanásia ocorreu no 14º dia do pós-operatório. Na pesquisa avaliou-se: processo inflamatório, proliferação fibroblástica, colágeno, neovascularização, epitelização macro e microscópica das cicatrizes. RESULTADOS: Observou-se presença de eosinofilia em todas as cicatrizes expostas ao cianoacrilato, e aumento significativo da neovascularização no subgrupo B2 em comparação com o A2 (p=0,037). Demais variáveis não apresentaram diferença estatística. CONCLUSÕES: A nicotina não influenciou o processo inflamatório, a proliferação fibroblástica, a presença de colágeno e a epitelização. A neovascularização indicou imaturidade cicatricial na comparação dos grupos A2 e B2. A eosinofilia nas cicatrizes reparadas com cola indica que a substância atuou como alergeno.

Background: Some of the disadvantages of calcium hydroxide Ca(OH) 2 as pulp-capping material are related to the inflammatory response, and its poor sealing ability. Cyanoacrylate glue was proposed in this study as pulp-capping agent because of its sealing ability, and diprogenta, on the other hand, was proposed because of its anti-microbial and anti-inflammatory effects. The aim of this study is to evaluate and compare the histopathological response of the pulp toward cyanoacrylate and diprogenta in direct pulp capping (DPC). Materials and Methods: Cyanoacrylate, diprogenta, and calcium hydroxide were applied on 20 permanent teeth of 6 rabbits divided into four groups, each contains five teeth, as follow: G1: Diprogenta and calcium hydroxide. G2: Cyanoacrylate. G3: Diprogenta and cyanoacrylate. G4: Calcium hydroxide (control group). Results: A remarkable dentin bridge formation was found in all groups. No pulp necrosis was found in any tooth of the four groups. Pathological reactions of the pulp were found in 25%, 40%, 20%, and 100% of groups 1, 2, 3, and 4, respectively. Differences between study and control groups were found statistically significant ( P < 0.05). Conclusion: All materials used in this study were biocompatible and suggested to be studied further on human teeth. These materials when used in combination with Ca(OH) 2 , may give better results

BACKGROUND: The advantages of the cyanoacrylates in cutaneous synthesis have been often demonstrated in the literature. However, these products have been underutilized in Brazil due to the high costs of the 2-octil-cyanoacrylate. Besides, few studies have been done with the more economically accessible form, the 2-etil-cyanoacrylate, as a cutaneous tissue adhesive. OBJECTIVE: To evaluate the effectiveness of the closing of cutaneous lesions using ECA. METHOD: This was a prospective study in which 46 wounds were occluded using the low cost ECA as an alternative to intradermal suture. RESULTS: Excisions (97,8%) and traumatic wounds (2,2%) were treated with 2-etil-cyanoacrylate and deep relaxed sutures as synthesis method. Unaesthetic scars (22%), infection (2,1%), dehiscence (2,1%) and contact allergic dermatitis (2,1%) were the problems we found. There were no cases of necrosis or keloids. The results were considered satisfactory in most cases (97,3%). CONCLUSION: The use of 2-etil-cyanoacrylate was shown to be safe and with satisfactory cosmetic results in this group of patients.


FUNDAMENTOS: As vantagens dos cianoacrilatos em síntese cutânea têm sido sobejamente mostradas na literatura. Entretanto, estes produtos têm sido subutilizados no Brazil devido aos elevados custos do 2-octilcianoacrilato. Ademais, a forma mais economicamente acessível, o 2-etilcianoacrilato, tem sido pouco estudada como adesivo cutâneo. OBJETIVO: Avaliação da eficácia do fechamento de lesões cutâneas usando o 2-etilcianoacrilato. MÉTODO: Estudo prospectivo no qual 46 feridas foram ocluídas usando o 2-etilcianoacrilato de baixo custo como alternativa a sutura intradérmica. RESULTADOS: Feridas excisionais (97,8%) e traumática (2,2%) foram tratadas com 2-etilcianoacrilato e suturas profundas relaxadoras como método de síntese. Cicatrizes inestéticas (22%), infecção (2,1%), deiscência (2,1%) e dermatite alérgica de contato (2,1%) foram os problemas encontrados. Não houve casos de necrose ou quelóides. Os resultados foram considerados satisfatórios na grande maioria dos casos (97,3%). CONCLUSÕES: O uso do ECA mostrou-se seguro e com resultados cosméticos satisfatórios neste grupo de pacientes.

Objetivo: O objetivo deste estudo foi avaliar as propriedades antimicrobianas do etil-cianoacrilato (Super Bonder®).Materiais e métodos: O método utilizado foi o de Difusão em meio sólido – orifício em agar contra S.aureus, S. mutans, S. oralis, S. epidermidis, E. faecalis, E.coli and B. subtilis. A ação bacteriostática foi observada através da medi-ção dos halos de inibição e a bactericida através do repique.Resultados: Todos os microrganismos foram inibidos na presence do Super Bonder®, apresentando halos que variaram de 2 a 12 mm. Porém, todos apresentaram crescimento após o repique, o que significa que o Super Bonder® é apenas bacteriostático.Conclusão: foi concluído que o Super Bonder® (Loctite) apresenta propriedades antimicrobianas na inibição do crescimento por contato
